Very clever, I would assume 2 separate shots done in a similar lighting setup, then composited. Looks as though the paper elements were cut to approximate the shape of the face, much easier to do real then in photoshop. Then again paper, might be done in 3D because there are not many pages.
After checking out his website, it looks as though he did a combination of all 3. Probably took 2 separate shot like Wonderdude mike said and modeled a series of pages. Then probably photoshopped it all together. If thats not how he did it then Im clueless :P
Its an amazing piece though, I think i did a double take when i saw this.
the wrinkles on his face really sell it for me :)
